α-methylacyl-CoA racemase is a protein with a molecular weight of 42 kDa, located in the cytosol, peroxisomal matrix and cytoplasm of cells. In humans, α-methylacyl-CoA racemase reacts with signaling receptor, and in addition, has been thought to be involved with essential functions such as CoA-transferase and α-methylacyl-CoA racemase activity. α-methylacyl-CoA racemase is thought to be essential to organismal processes, namely, bile acid biosynthetic process, protein targeting to peroxisome and fatty acid β-oxidation using acyl-CoA oxidase. α-methylacyl-CoA racemase is clinically significant because abnormalities in its function have been closely linked to diseases like α-methylacyl-CoA racemase deficiency (AMACRD) and congenital bile acid synthesis defect 4 (CBAS4). Congenital bile acid synthesis defect 4, an autosomal recessive inheritancedisorder characterized by elevated hepatic transaminases, hyperbilirubinemia and failure to thrive, has in particular been of interest to investigators.
